The tide of MFAs in Schleswig-Holstein has been ebbing, leaving medical practices floundering in the daily storms of patient care. As reported by doctors' representatives, this workforce crisis is making a deep cut into the operations of these institutions.

By mid-last year, there were approximately 14,929 MFAs swabbing their decks in Schleswig-Holstein's medical practices, according to the Association of Medical Professions. But alas, the Association's president, Hannelore König, warns that this number has been in a steady decline since 2021.

Many MFAs are jumping ship due to why, you ask? It's simple: the pay is pathetic compared to the weighty responsibility they carry for patient health, the daily grind, and other career paths in related fields, König explains.

"Without MFAs, a practice is almost nation!"

So, what are MFAs? Well, they are the backbone of a medical practice. Jens Lassen, the House Doctors' Association Chairman of Schleswig-Holstein, puts it plainly: "Without MFAs, a practice is almost nothing!" The role is exciting, challenging, and has evolved considerably in recent years. But therein lies the rub. You think there's a dearth of MFAs now? Aye, there be many practices in Schleswig-Halstein hankering for fresh blood, but Lassen couldn't provide exact numbers - shivers!

Why, you ask? Well, the profession's growing pains, my friend, are causing a strain on the workforce. The shortage of MFAs is directly translating into reduced practice performance, impacting patient care in the worst way possible: prolonged waiting times, curtailed services, and more work loading on physicians.

"This is a pressingly critical problem..."

"We feel the pinch," says Delf Kröger, the Association of Statutory Health Insurance Physicians Schleswig-Holstein spokesman in Bad Segeberg. "It's tough for doctors to find top-notch MFAs. Skilled assistants are the lifeblood of a practice."

The staff shortage emphasizes every specialty, and to complicate matters further, doctors lose their diamonds in the rough to competitors promising greener pastures - usually, plump hospitals offering juicier deals. "Consequently, we demand increased funding for medical practices. They must be able to pay well-deserving, skilled MFAs," Kröger exclaims.

The prospect of even worsening shortages looms ominously. According to the Federal Employment Agency, four out of six criteria indicate a shortage of MFAs in the Schleswig-Holstein/Hamburg district. The job-seeker-to-vacancy ratio has been steadily decreasing, and only recovers after final exams. Batten down the hatches, folks, because Schleswig-Holstein has had this ratio above 1.1 for several months in 2024!

König prophesizes that the tide of MFAs will continue to ebb. "Because on the one hand, the baby boomer generation is drawing closer to retirement, and on the other hand, more MFAs are needed to take the burden off our doctors."

As long as the competition for MFAs between medical practices and hospitals remains lopsided, the shortage in medical practices will continue to worsen, especially in larger cities and areas near hospitals.

The Association of Medical Professions has managed to secure significant wage increases for MFAs over the past few years. But the gap between MFAs' pay and that of comparable professions remains considerable. MFAs undergo the same three-year training as nursing professionals and carry the same patient-related responsibility, says the president.

"This salary disparity must be bridged," Hannelore König declares. "A breakthrough in this area, as with nursing, requires that collective wage increases be co-funded, fully considering the financing of services provided by practicing physicians."
